The Insuree’s Rights

Procedure and documentation required

The procedure for exercise of the right to pension and disability insurance is inicitated:

– at the request of the insuree for exercise of the right to old age pension
– at the request of a family member of the deceased insuree or pension beneficiary for exercise of the right to family pension
– at the request of the insuree or on a proposal from a medical doctor treating the insuree, or by a medical commission for exercise of the right to disability pension (disability pension, injury-based financial benefit)
Application for exercise of the right to pension and disability insurance is submitted by filling in EP-10 Form (application form for exercise of the right to pension and disability insurance), attachment 1, to a regional unit of the Professional Service of the Pension and Disability Insurance Fund where the insuree was last insured.
