Documentation required for exercise of the right to pension and disability insurance is the following:
a) For old age pension:
– work record booklet and evidence of the pension length of service that is not registered therein (decisions, certificates, confirmations and etc.)
– birth certificate with personal identification number or insight in ID card
– decision on termination of employment, or termination of insurance (М2 Form).
b) For the right to disability insurance:
– work record booklet and evidence of the pension length of service that is not registered therein (decisions, certificates, confirmations and etc.)
– birth certificate with personal identification number or insight in ID card
– Form No 3 issued by a family doctor, (Proposal for verification of the disability with a finding and opinion on the insuree’s health condition and working ability)
– medical documentation prescribed by the Decision on compulsory minimum of the range and content of medical documentation in the procedure for exercise of disability-based rights (Official Gazette of RM No 88/04).
– workers compensation claim form(if the insuree suffered injury at work).
c) For the right to survivor pension:
– work record booklet and evidence of the pension length of service that is not registered therein (decisions, certificates, confirmations and etc.) if the right to survivor pension is exercised following death of active insuree.
– birth certificate with personal identification number of all family members for which approval of the right to survivor pension is requested.
– marriage certificate for the widow-widower for whom approval of the right to survivor pension is requested (the extract is to be issued upon the death of the insuree – pension beneficiary)
– death certificate and notification of the accident at work if the death suffered work injury.
– evidence for regular education of children older than 15 for whom approval of the right to survivor pension is requested
– medical documentation for family members for whom approval of the right to survivor pension is requested based on working inability
– evidence for adoption, for joint household, household property, means of subsistence, military service and etc. in cases where for particular family members the approval of the right to survivor pension is conditional to existence of certain facts.
In addition to the above evidence, the application for approval of the right to pension and disability insurance may be accompanied by other evidence to determine facts related to exercise of the rights (decisions for use of some of the rights to pension and disability insurance, court alimony decisions, certificates-confirmations for employment or business practice and etc.)
